Gym Membership /Membership Add-On /Premium Service
Cancelation Form
It is my intent to cancel my Gym Membership, Membership Add-On, or Premium Service with Level 11 Fitness & Development, LLC . I understand this notice is being given 30 days prior to my intent of cancelation, thus I will be charged for one more month's worth of membership.
My monthly membership cost was determined by my original contract agreement. I understand that cancelation of membership before my initial commitment is completed holds a buy out fee; it is my responsibility in signing my contract to Level 11 Fitness & Development, LLC that I will meet this commitment.
If I have already completed my initial commitment, I will not be required to buy out my membership.
Access Cards : I understand by signing this document I have 30 days to return my access card either in person or by mail to 2708 Akron Road, Wooster OH 44691. Failure to do so will result in a $10 access card fee, as agreed upon in my initial contract. IF RETURNING ACCESS CARD IN PERSON: Place card in black lock box outside of front office inside the gym.
I understand the banking information on file with Level 11 Fitness & Development, LLC will be charged the respective amounts above.
