ALLERGY WAIVER
All brushes and makeup products are kept sanitary and are cleaned between every makeup application. Any skin condition, or contraindication should be reported by the client to the Makeup Artist/Hairstylist prior to services. If requested, skin patch test may be performed to test reaction. Artists/Stylists will to the best of their ability refrain from using any products that contain allergens listed below, however Client(s) agree to release the makeup artist / hair stylist from liability for any skin complications due to allergic reactions from any product applied. Client(s) understand that they will be in direct contact with various cosmetics and products topically applied as well as aerosol hair sprays, and hair products.
