The attached Client Screening and Liability Waiver forms must be completed prior to service.

WASH HANDS

IMMEDIATELY UPON ENTRY
All guests must wash hands for 20 secs when entering salon.
MINIMIZE PERSONAL BELONGINGS
Leave large purses/bulky items in your car. This will minimize the surfaces your belongings will come in contact with.
MASKS & GLOVES (PPE)
Personal Protective Equipment, such as masks, are to be worn at all times. Gloves are not required, but are highly recommended.
SOCIAL DISTANCING
All guests must practice social distancing. (i.e. 6 feet apart)

In order to be in compliance with the health and safety rules as suggested by the government, it is mandatory that you are not allowed to enter the salon if you answer yes to ANY of the following questions or are experiencing or have experienced any flu- like symptoms in the last 14 days.

COVID-19 RISK LIABILITY WAIVER
We all desire a safe salon environment and we all have a role in ensuring safety for all. It should be understood and known up front that there are risks involved when entering ANY estabishment. We have no way of knowing who has been exposed or infected with COVID-19. We will not service anyone exhibiting known signs of COVID-19. WE are servicing you with a risk of being exposed and YOU are offering to be serviced with a similar risk of exposure. Awareness and honesty are important to help lower the risk of exposure and spread of COVID-19.

As a Guest, I am choosing to be serviced at my own risk of being exposed. The salon, hairstylist, or staff are not liable for any possible exposure that I may have with any surface or individual(s) that may be contagious with COVID-19 or any other infection or disease. I am assuming all risk associated with entering a public establishment.

I agree to comply with wearing a PPE facemask. I will adhere to social distancing, hand washing procedures. I will not enter this facility knowing I have been exposed to COVID-19. I agree to following health and safety practices, as suggested by CDC and local government officials.
