Castaways Foster Application Form

Castaways Rescue is an all-volunteer, veteran owned non-profit, dog welfare organization dedicated to the rescue and placement of stray, neglected and abused dogs. Castaways Rescue is completely funded through fundraising efforts and public donations. All of our pets are cared for by foster homes, and as such, fosters are critical to our continued ability to save and place dogs. We sincerely appreciate your application and willingness to open your heart and home to a Castaways dog.

By sending this electronically, I certify the above information is correct, and I acknowledge that I have completely read this questionnaire and comprehend it fully.

I accept full responsibility for the dog(s) actions at all times, and release Castaways Rescue, Jones County, the Sheriff, and their employees, agents, and officials of any and all manner of claims, demands, actions, causes of action, suits, debts, expenses (including reasonable attorneys’ fees), sums of money, promises or damages whatsoever, in law or in equity, known or unknown, whether heretofore asserted or not, arising directly or indirectly out of or related to the fostering of such dog(s).

I understand that in any case I am found neglible in the care of a dog resulting in unforseen medical expenditures, I may be held liable for veterinary expenses, i.e. preventable accidents or injuries due to improper supervision, non-compliance with special instructions, and/or third party negligience transpiring from events taking place without prior notification to Castaways.

I agree to have Castaways Rescue complete reference call checks and conduct a home visit inspection to be able to approve my foster application.

Castaways Rescue Obligations:

1) Pay all fees for the dog's scheduled care at a Castways approved veterinary clinic.

2) Continuously evaluate Foster/dog relationship.

3) Find alternative arrangements for the dog if the foster is no longer able or willing to care for the dog.

4) Provide the Foster with all relevant information available concerning the dog's health and temperament.
