Deposit
A $15 deposit is necessary to confirm your V-steam appointment. You may also pay in full but $15 is not refundable if cancellation policy is not upheld. (Payment methods on bottom of form)
Availability
Due to my regular weekly schedule I am only available for scheduling appointments Thursday evenings, Fridays, Saturdays and a few hours on Sundays. Thank you.

Cancellation Contracts
*If you have been given a FREE session and you schedule an appointment and miss it for whatever reason the free session is REVOKED. *Cancellation requests/rescheduling will not be honored for FREE sessions, so please be sure to show up to your ($50 value) free session. *Once this form is submitted and date confirmed rescheduling will not be available for FREE sessions. *Paying clients are subject to the 24hr cancellation agreement.
24hr Cancellation Contract
You must cancel 24 hrs prior to appointment to receive full refund. Last minute cancellations will be $15 set up fee for no show.
