CAMP INFORMATION

CitySports Soccer Summer Camp is an outdoor, one week children's soccer day camp located in Bayonne, NJ. Welcoming children ages 6 to 14 years old, this camp is designed to effectively teach your child the fundamentals of the beautiful game all while keeping them consistently entertained and engaged. The camp will operate for five weeks from 9AM-3:30PM, Monday-Friday. Half-days will be available from 9AM-12:30PM. Before care will be held free of charge starting at 8AM. Aftercare will be offered from 3:30-5:00PM, at an additional cost. Each day will consist of different soccer training techniques, lessons, games, and an assortment of free time activities to ensure an optimal experience for each child.

Our campers safety and health is our number one priority: as we continue to follow the guidelines of the state of NJ, the CDC, and NJ Youth Soccer we guarantee adjustments will be made to meet any new guidelines set forth. Rules suggest that a facility (soccer field and immediate area) can have up to 25 people total. This number may be adjusted based on recent Governor Phil Murphy's announcements. Please keep in mind there are limited spots available.  Upon completion of this form, you will receive an email with a waiver and full details of our camp protocols. Please be prepared to print, sign, and return the necessary forms at the time of your child's first drop off. Failure to do so will result in your child's dismissal from the camp until a physical copy is obtained by the Camp Director.

To ensure the safety of each child, we request that all meals, drinks, and snacks be prepared and brought from home. CitySports Arena will offer water and gatorade daily, this will be served additional to each childs personal supply. All campers are required to wear masks at the time of check-in. Each camper will have their tempurature taken upon entry and be instructed to sanitize before addmittance. Any camper with a tempurature above 100ºF will not be permitted to enter and must provide documentation of medical clearance to participate upon return.

INFORMED CONSENT & ACKNOWLEDGMENT

I hereby give my approval for my child’s participation in any and all activities prepared by CitySports Arena during the selected camp. In exchange for the acceptance of said child’s candidacy by CitySports Arena. I assume all risk and hazards incidental to the conduct of the activities, and release, absolve and hold harmless CitySports Arena and all its respective officers, agents, and representatives from any and all liability for injuries to said child arising out of traveling to, participating in, or returning from selected camp sessions.

In case of injury to said child, I hereby waive all claims against CitySports Arena including all coaches and affiliates, all participants, sponsoring agencies, advertisers, and, if applicable, owners and lessors of premises used to conduct the event. There is a risk of being injured that is inherent in all sports activities, including soccer. Some of these injuries include, but are not limited to, the risk of fractures, paralysis, or death.

MEDICAL RELEASE & AUTHORIZATION

As Parent and/or Guardian of the named athlete, I hereby authorize the diagnosis and treatment by a qualified and licensed medical professional, of the minor child, in the event of a medical emergency, which in the opinion of the attending medical professional, requires immediate attention to prevent further endangerment of the minor’s life, physical disfigurement, physical impairment, or other undue pain, suffering or discomfort, if delayed.

Permission is hereby granted to the attending physician to proceed with any medical or minor surgical treatment, x-ray examination and immunizations for the named athlete. In the event of an emergency arising out of serious illness, the need for major surgery, or significant accidental injury, I understand that every attempt will be made by the attending physician to contact me in the most expeditious way possible. This authorization is granted only after a reasonable effort has been made to reach me.

Permission is also granted to CitySports Arena and its affiliates including Directors, Coaches, and Team Parents to provide the needed emergency treatment prior to the child’s admission to the medical facility.

Release authorized on the dates and/or duration of the registered season.

This release is authorized and executed of my own free will, with the sole purpose of authorizing medical treatment under emergency circumstances, for the protection of life and limb of the named minor child, in my absence.

CONFIRMATION

BY ACKNOWLEDGING AND SIGNING BELOW, I AM DELIVERING AN ELECTRONIC SIGNATURE THAT WILL HAVE THE SAME EFFECT AS AN ORIGINAL MANUAL PAPER SIGNATURE. THE ELECTRONIC SIGNATURE WILL BE EQUALLY AS BINDING AS AN ORIGINAL MANUAL PAPER SIGNATURE.
