APPOINTMENT REQUEST

 ​All PAWS Clinic services are by appointment only.

 

If you are experiencing an emergency, please contact your primary veterinarian or the emergency clinic.

If you do not qualify as low-income, please understand that the fee will be higher for you, but we are happy to see your pet if you do not have access to a veterinarian. 

Please note: To secure your appointment spot, the PAWS Cares Community Veterinary  Clinic requires a non-refundable deposit that will go towards your total bill at checkout.

Reschedules and cancellations must be made at least 72 hours prior to your scheduled appointment time.  If you fail to do so you will forfeit your deposit.

PAWS reserves the right to refuse service to animals we believe are too aggressive or pose a risk to our staff, and/or other animals. 

If you have any medical records, please bring them with you.

* Spay - $225.00  ** Additional charges may apply for Pyometra & in-heat
* Neuter - $175.00  ** Additional charges may apply for Cryptorchid
* Rabies vaccination - $10.00 (required if your pet is not vaccinated)
* Microchip - $25.00 (no additional registration fees are required)

​DOGS - Under the age of 5  - Under 40 pounds only

All dog procedures require a pre-surgical exam ($40) and 4DX test ($40) prior to the surgery.

* Spay - $350.00-450.00  Fee varies by weight 

     ** Additional charges may apply for Pyometra & in-heat

* Neuter - $250.00-350.00  Fee varies by weight 

** Additional charges may apply for Cryptorchid

*  Rabies vaccination - $10.00 (required if your pet is not vaccinated)
*  Microchip - $25.00  (no additional registration fees are required)

If your pet has fleas and/or ear mites we will treat for this and you will be charged as follows: 

*  Fleas - $10.00 for Cats / $15 for Dogs
*  Ear mites - $10.00

A deposit is required when you complete this form. All other fees are due at the time of pick-up.  CASH or CREDIT CARD ONLY - NO CHECKS will be accepted

​Should you fail to show up for your appointment (no call/no show) your deposit will not be refunded.

​Deposit Fees:

Deposit fees are non-refundable if appointment is cancelled within 7 days of the appointment.

** Please note that processing fees will be deducted from your refund.

​DOGS - All dogs MUST be under 40 pounds

Female (Spay) & Male (Neuter) $150.00

The deposit will be deducted from your total bill.

TERMS, CONDITIONS, & WAIVER OF LIABILITY:

The PAWS Cares Wellness Clinic uses qualified staffing and approved materials for all procedures performed. It is important for you to understand that the risk of injury or death, although extremely low, is always present just as it is for humans who undergo surgery. Carefully read and understand the following before signing your name.

Acting as owner or agent of the pet named at the top of this form, I hereby request and authorize the PAWS Cares Wellness Clinic, through whomever veterinarians they may designate, to perform an operation for sexual sterilization of the previously named animal.

• I understand that the operation presents some potential hazards, and that injury or death of such an animal may conceivably result, for there is some risk in the procedure and the use of anesthetics and medications in providing this service. 
• I certify that my animal is in good health and, if over 4 months old, has had no food since 12:00 midnight the evening prior to surgery.
• I understand that the PAWS Cares Clinic has the right to refuse service to any animal to whom surgery is deemed a health risk.
• I understand that the PAWS Cares Clinic performs just a brief physical examination before surgery.
• I also understand that my animal will not receive pre-operative bloodwork and I waive my right to have this service performed at a full-service veterinarian prior to surgery.
• I understand that some factors significantly increase surgical risk, including but not limited to: pregnancy, heat, and diseases such as feline immunodeficiency virus, feline leukemia.
• If my animal has an open and small umbilical hernia, it will be repaired at the time of surgery. If my animal has one or more cryptorchid testicles, these will be removed. If another medical condition presents itself during the surgery, and the Veterinarian can address the problem, I understand there will additional charges. The cost of these additional services ranges between $15-$205.
• I agree to pay any additional balance due at pick-up the day of surgery.
• I understand that if I don’t retrieve my pet at the agreed upon time that PAWS Cares Clinic exercises its right to either assume legal ownership of the animal and prepare for adoption or handle as deemed just and proper as allowed by the State of Maine.
• I agree to pay a $15 per quarter hour late fee in the event that I am more than 15 minutes late for my scheduled pick-up time. In the event of a medical emergency, I authorize the PAWS Cares Clinic to seek care at, and transport my pet to, a full-service veterinary facility or board at the shelter if space is available.
• I understand that all fees are due at the time of pick up. Cash or Credit Cards only. 
• I understand that a 3.9% fee will be charged on all credit card transactions.
• I understand that if I fail to show up for my scheduled appointment that my deposit will not be refunded.

I hereby release the PAWS Animal Adoption Center, the PAWS Cares Clinic, all veterinarians, assistants, volunteers, directors, and employees from any and all claims arising out of or connected with the performance of this procedure or any adverse reactions from vaccinations. I agree that I have not and will not claim any right of compensation from them, or any of them, or file action by reason of such sterilization or attempted sterilization of such an animal or any consequences related thereto. I the owner/agent herby agree to indemnify and hold PAWS Animal Adoption Center harmless for any such damages caused during the transportation of the animal, or for any damages caused by unforeseeable events including fire, vandalism, burglary, extreme weather, or natural disasters.

I give permission for my pet’s medical records to be released to my veterinarian.

Once we recieve your form and deposit our Clinic Coordinator will contact you to schedule your pets spay/neuter appointment.  Appointments are scheduled on a 1st come, 1st served basis.  

We reserve the right to refuse service to any animal deemed to be dangerous to our staff or should the requested procedure pose a risk to your pet.
