    Hopewell Veterinary Hospital

  • Pets are typically fed twice a day in the morning and in the evening.  We suggest bringing your pet's typical food from home.  Dogs that do not have their own food will be given Hill's Sensitive Stomach dry food.  Cats that do not have their own food will be given Hill's cat food.  Pets other than dogs and cats (birds, reptiles, small mammals) must bring own food and their cage/house.

  • If you are dropping off more than 4 pets, please sign, hit submit and start a new form for additional pets!

  • In case of illness or injury, I do hereby give my consent for Hopewell Veterinary Hospital to treat, prescribe for, or operate upon my pet(s) while they are being boarded.  The hospital is to use all reasonable precautions against illness or escape of my pet(s), but the hospital will not be held liable under any circumstances on account of the care, treatment or safekeeping of my pet(s) as it is thoroughly understood that I assume all risks with regard to disease or injury, including those associated with aged pets.

I understand that by PA state law, all dogs are required to have a license (available through county of residence). If my dog is not licensed, I understand that I am responsible for payment of the fine and costs that may be charged for my pet in the event that Hopewell is auditted for dog licenses during my pet's stay.

Should circumstances arise that my pet(s) remain unclaimed after the date which I have stated as the pick-up date, I understand that written notice will be mailed.  Fourteen days after such written notice, the pet(s) will be considered abandonded.  It is further c that such action will not relieve me of paying the costs of all your services, including the cost of boarding.

I understand that Hopewell Veterinary Hospital does not allow personal items other than food or medication to be left with my pet(s).  Hopewell Veterinary Hospital is not responsible for any such items.  For the safety of your pet(s), we do not allow choker or prong collars to be left on dogs.
