MSJ Registration Form

Dear Parents/Guardians:

Thank you for your support during the current academic year. In order to plan for the 2012-13 academic year, we now need to begin the re-registration process. Please complete this form and return it with the registration fee to Betsy Morgan in the main office at Christ the King, or to Registrar’s Office at Mount St. Joseph Academy. This year, you may also submit your registration form and payment via credit card through the schools’ websites: http://www.cksrutland.org/ or http://www.msjvermont.org/.

Registration Fee: $125.00 per student, not to exceed $250 per family, from January 12th through February 29th. Beginning March 1st, the fee is $175 per student, not to exceed $350 per family. The registration fee is separate from tuition, non-refundable and must be paid at the time of registration.

FUNDRAISING AGREEMENT
Fundraising CKS

I understand that I will be obligated to participate in three to four major fundraisers that help make up the difference between tuition charged and the actual per pupil cost ( there will be a partial “opt out” option – details available from the PTC)I do not wish to fundraise and therefore agree to pay $400.00 per family toward the tuition difference, payable by separate check to CKS PTC by September 10, 2012. I understand that I may still participate in any of the individual fundraisers if I so choose.

I understand that I am needed to volunteer for at least two or three PFC, Development, and Marketing events in order for MSJ to make up the difference between tuition charged and the actual per pupil cost.

Credit Card payment is an option provided to you as a convenience by Rutland Catholic Schools; therefore, a 2% Convenience Fee has been added to your payment.

Payment by personal check or money order can be made to: Rutland Catholic Schools, 127 Convent Ave., Rutland VT 05701. There is no convenience fee associated with a cash or check payment.

The early registration fee is $125.00 per student (plus convenience fee), with a maximum of two registration fees per family. The registration fee will be $175.00 (plus convenience fee) after February 29, 2012.
