Returning Athlete Consent Form

Our records indicate that your student athlete has a completed Athletic Handbook on file. Each year we are required to have a current physical (the physical is good for one year from the date the physical was given) on file along with the Athletic Handbook. If nothing has changed from the previous year with the Athletic Handbook, it is not necessary to complete a new one. It is the paret/guardian's responsibility to report any change in medical coverage/insurance. Please indicate your player's name and sign that everything is up to date.
