2024-2025 Caregiver Product Agreement Form
Welcome to the Girl Scouts of Western Ohio Product Programs!

For contact-free ease, we have moved to an online agreement form. Please complete, granting your Girl Scout permission to participate in the Fall Product and/or Cookie Program(s).

Should your Girl Scout be a part of multiple households, each caregiver will need to complete their own agreement to avoid any liability disputes.

2024 Fall Product Program Dates:
- Program Begins: October 4
- Last Day for Girl Delivered Orders: October 29
- Fall Product Delivery to Troops: November 7-9
- Program Ends and Last Day for Shipped Orders: November 10
- Check with your fall product manager for order and money due dates
2025 Cookie Program Dates:
- Order taking Begins: January 3
- Cookie Deliveries to Troops: February 6-13
- Cookie Booths Open: February 14
- Last Day for Girl Delivered Orders: March 6
- Program Ends and Last Day for Shipped Orders: March 16
- Check with your troop cookie manager for order and money due dates

Please use this agreement for the following Girl Scout Product Programs*

MONEY & RECEIPTING PROCESS
- Money from customers is collected when cookies are delivered, except for online orders, which are already paid by credit card at the time of the order.
- If accepting a check, customers can make payable to your troop. Example: Girl Scouts of Western Ohio Troop 12345 or GSWO Troop 12345.
- Do not write one personal check to the troop for all your orders.
- Do not keep large amounts of cash as you are responsible for any funds, including any lost or stolen. Make frequent visits to your troop fall/cookie manager to turn in cookie payments.
- Obtain a signed receipt anytime funds or cookies are exchanged between yourself and your troop volunteers.
- Keep in mind, product cannot be returned or exchanged. You are financially responsible for all product ordered.
- Any donations from customers should be accounted for and turned in with your other deposits.

PERMISSION TERMS

I agree to the following:
- I will ensure that my Girl Scout has adult guidance at all times.
- My Girl Scout will not take orders until the program(s) begins.
- I will not collect payment from a customer until the product is delivered.
- When exchanging product or funds to/from troop volunteer, I will obtain a copy of the signed receipts.
- I will submit all funds received for payment from sales to the troop fall/cookie manager by their required deadlines.
- All product obtained will be picked up and paid for no later than the program(s) end date.
- I understand that I cannot return product once they are ordered by my troop, and I agree to accept full financial responsibilities for all of the product I order. If I am in need of assistance with selling any remaining product, I will contact the troop volunteers right away.
- I understand that should any product or money become lost, stolen or damaged, I am still fully responsible.
- I understand that if I do not pay for the full amount due by troop deadline, I could be subject to collections and/or legal prosecution including any fees.
COVID-19 Statement

I acknowledge that COVID-19 is an extremely contagious virus that spreads easily in the community. I agree to adhere to Girl Scouts of Western Ohio and state and local guidelines and mandates. I will take all reasonable precautions to limit potential exposure for girls, volunteers, and families, based on Girl Scouts of Western Ohio and state guidelines. I will hold Girl Scouts of Western Ohio harmless and waive all right to legal action, if my daughter contracts COVID through exposure at a Girl Scout event.
