GROUP BOOKING FORM

*PLEASE READ BEFORE FILLING THIS OUT! IMPORTANT INFORMATION*

To reserve seats please complete and submit the booking form.

We ask that everyone reades the Contract below and acknowledge by signing and also as in the past ask for $50 deposit which can be paid through Paypal, Credit Card, or by Mailing a check to: Bear Wallow Farm, PO Box 242, Nancy, KY 42544.

Booking will be held for 1 week following submission. After 1 week, if deposit has not been received (or plans made to get deposit to us) we will be forced to give that booking to the next person in line.

The clients wish to hold an event at Bear Wallow Farm and agree to the following policies:

1.Reservation Fee- The deposit must be received by Bear Wallow Farm before your event is reserved. The total reservation amount and additional guest admission must be paid on or before the event date, unless approved in advanced.

2.Date/Time Changes – If the clients wish to change the date or time of the event they may contact Bear Wallow Farm and request this change. Requests will be accommodated if possible but no guarantees are made. If request cannot be accommodated, the event will proceed as scheduled. A denied request for date/time change after booking is not grounds for refund.

3. Outdoor Location- The client understands they are booking an outdoor venue where we cannot control weather events. In order to receive a refund for your event due to weather it must be raining AT Bear Wallow Farm at the time your event is scheduled to take place.

4. Unforeseen Events- The client shall not hold Bear Wallow Farm responsible for failure to provide necessary facilities in the event of power outage, water loss or acts of God.

5. Food and Catering- The clients may contract with Bear Wallow Farm to provide food or supply the event themselves. We do not have a public kitchen. If food is not provided by Bear Wallow Farm clients are responsible for set up and cleanup of said food. There are no outside grills allowed and there are no outlets in the shelters!

6. Liability- The clients shall not hold Bear Wallow Farm responsible for loss or injury.

7. Alcohol & Drugs- Alcohol and drugs are prohibited. Anyone found in violation will be escorted from the grounds and denied readmission. No exceptions.

8. NO PETS ALLOWED- ALL Service animals must have proper documentation.

9. The date/time/booking type that you select can only be changed if still available at request. Please be sure to select the correct TIME, DATE, and TYPE of booking on this form. Saturdays have 2 different booking times available (10AM - 4PM OR 5PM-9PM) If you have questions, concerns, or are intested in catering at the farm (min. 200 people) please contact us at 606.875.5160 or email bearwallowfarm@hotmail.com

(if you do not reach the minimum amount of people you are still expected to pay minumum for the tables/shelter; likewise if you have more than the minimum amount of people you are still expected to pay for those people above the minimum at the discounted rate of $15.95 (plus tax) per person)

*Minimum cost for Tables only in the field is $398.75 (plus tax).  This includes first 25 admissions. Any admissions above the 25 is still $15.95 per person.

*Minimum cost for Large Shelters is $638 (plus tax)and includes first 40 admissions.  Any admissions above the 40 people is still $15.95 per person.
