SOT is a transport rescue. I understand all healthy dogs will be adopted out of state. Heartworm positive dogs and specific breeds might remain in Texas at discretion of board of directors. By typing yes, I understand I may not adopt or keep my foster.

By sending this electronically, I acknowledge that I have completely read this questionnaire and comprehend it fully.

I understand that applying does not ensure approval and that untruthful answers or failure to comply with the requirements of this application can result in the forfeiture of any Straight Outta Texas animal fostered by me.

I certify that the above information is correct, and I understand that the information will be verified.

I understand that by submitting this form electronically, I agree to release and covenant to hold harmless Straight Outta Texas and it's members from any claims, damages, costs, or actions incurred because of the care or actions of the foster dog.

I accept full responsibility for the dog(s) actions at all times, and release Straight Outta Texas from any liabilities or damages that may be incurred because of fostering such dog(s).

I agree to have Straight Outta Texas complete reference call checks and conduct a home visit inspection to be able to approve my foster application.

I agree that if I'm unable to foster the dog(s) anymore that I will return the dog(s) to Straight Outta Texas and try to give Straight Outta Texas a 1 week period to try and find another suitable foster for the dog(s). I agree I will not leave my foster dog(s) in anyone elses care, that is not approved by Straight Outta Texas.

I understand that the foster dog(s) belongs to Straight Outta Texas and that I do not have the right to keep, give away, sell, or adopt out the dog(s).

I agree to be patient with the foster dog(s) and provide the foster dog(s) with food, fresh water, shelter, love and care.

I agree to send Straigh Outta Texas, or its members pictures and/or videos of my foster dog(s) at least once a week, and pass on all information I have learned about the dog(s) while in my home/care. To help ensure that Straight Outta Texas can find the best possible home for the dog(s).

I agree to notify Straight Outta Texas Inc, or its founders IMMEDIATELY if there is a health, medical issue, or injury concerining my foster dog(s). Or if my foster dog gets loose or is missing.

I agree that I will bring my foster dog(s) to any and all Vet appointments made by Straight Outta Texas.

I understand that I am obligated to bring my foster dog(s) to the transport site the day they transport. Or pick up my foster dog from the pick up location the day of arrival. If i am unable to do so due to work schedule, distance etc. I agree to notift Straight Outta Texas or its members right away, so volunteer transport help can be arranged.

I agree to give my foster dog(s) any and all medications/preventitives given to me by the Vet, or by Straight Outta Texas Rescue.

I understand that, if unvaccinated or undervaccinated, the foster dog(s) should not go out in public (the foster's yard is acceptable) or come into contact with any other dogs that are not fully vaccinated. Puppies are fully vaccinated once that have had 3 rounds of vaccines.
