Agreement & Release Form

In consideration of EMC Performing Arts Studio furnishing training and instruction for Student, we, the parents of Student, hereby agree and do hereby release, remise and forever discharge EMC Performing Arts Studio, its employees, agents, officers, directors, attorneys, successors or assigns, of and from all manner of, actions and causes of action, suits, debts, dues, accounts, bonds, covenants, contracts, agreements, promises, judgments, claims and demands whatsoever in law or in equity, especially arising out of, or in connection with the furnishing of dance or acro/gymnastic instruction, services and training and recreational services and training supplied by EMC Performing Arts Studio regarding Student, which against the said EMC Performing Arts Studio, its employees, agents, officers, directors, attorneys, successors or assigns, the undersigned or Student ever had, now have, or which their respective heirs, executors, successors or assigns, or any of them, hereinafter can, shall or may have, for or by reason of any cause, matter or thing whatsoever.

By the execution of this Agreement and Release, the undersigned hereby acknowledge and understand, since Students will be exercising or doing dance or acro/gymnastics or both at his or her own risk and since EMC Performing Arts Studio assumes no responsibility for any injuries or accidents which arise while Student is exercising or performing dance or gymnastics or both at EMC Performing Arts Studios it is extremely important that Student be in good health and physical condition since previous illness or injuries could be complicated by such physical exercise. The undersigned are further advised to consult their personal physician to determine if the Student should engage in such exercise of dance or gymnastics or both, prior to the commencement of such exercise of dance and/or acro/gymnastics program.

For safety reasons, please ensure that your children come to EMC classes in the proper dress code and with hair secured away from the face.

EMC Performing Arts Studio reserves the right to use photos and videos of all participants in promotional materials.
