Medical and Parent / Legal Guardian Consent form

For taking part in activities at Bushcraft Adventures
Bushcraft Adventures requires your consent for your child / children to take part in the activities.

Polite reminder about minimum age for activities:
Bushcraft & archery are age 5+
Bushcraft birthday parties are age 7+
Axe throwing is age 9+

We need your consent for each event that your child / children attends. You must let us know if any of these details change after submitting your form.

Your child / children will be participating in one or more of these activities, depending on the event you have booked or your child/children has been invited to. Please read and then complete & submit this form to give your consent for them to participate.
Bushcraft activities (e.g. fire lighting, shelter building, water purification and more), scavenger hunts and other outdoor/nature games, archery, throwing arrows, bushtucker trials (including edible bugs fit for human consumption), basic campfire cooking and/or using some basic tools such as potato peelers or knives, axe throwing (activities are age dependent) All activities are optional for children to take part in at the event.

We may take images (photos or video) at events for marketing purposes. These images could be used across print and digital media platforms including print publications, websites, e-marketing, posters, banners, advertising, film and social media. Please note that any images published on websites can be viewed across the world, not just in the UK. Do you consent for us to use images of your child / children in this way?

I, the undersigned: • Agree to my child / children taking part in the activities and agree that they will be expected to abide by the rules as laid down by Bushcraft Adventures; • Know of NO physical or medical reasons for my child / children not participating in any of the activities; • Understand that I must notify Bushcraft Adventures as soon as possible if any details change after submitting this form; • Agree that medical treatment will be given to my child / children if necessary and in the case of an emergency and that these details may be shared with the emergency services; • Am aware of the inherent risk involved in any adventurous activities and understand that even with all the safety measures in place, there is a risk of serious injury or death; • Understand that it is my responsibility to ensure that my child / children wear appropriate warm and waterproof clothing and closed footwear suitable for an outdoor environment and the activities; • Understand and accept that Bushcraft Adventures Ltd and its instructors do not accept any responsibility for any loss, damage or injury suffered by persons and/or their property arising out of or during the course of their activities unless such a loss or damage was caused by, or resulted from their negligence or a deliberate act; • Understand that the information I have provided on this form will be used for the purpose of this event only, it will be shared with Bushcraft Adventures instructors and will be stored securely for seven (7) years in accordance with Bushcraft Adventures’ Privacy Policy https://bushcraftadventures.co.uk/privacy-policy
Covid-19 statement - by submitting this form you confirm that you have read and accepted this statement
I understand that it is my responsibility to ensure that I am following current Welsh Government covid-19 legal requirements and guidelines, including the current rules on self-isolation: https://gov.wales/self-isolation Specifically, if: - my child / children or anyone in my household has a positive covid-19 test result; and/or - my child / children or anyone in my household has or has had symptoms of coronavirus within the last 10 days; and/or - we're awaiting test results; and/or - we are required to self-isolate (for example being advised by the TPP service or in line with Welsh Government guidance available here: https://gov.wales/self-isolation) I understand that we must not attend the event and must not enter Bushcraft Adventures' woodland.
