I acknowledge the total price can change if hair is different in person than what was discussed during the consultation, or if my wants/needs change during the appointment.

I agree to provide a credit card that will be kept on file.

I understand and agree to pay a $500 non-refundable deposit.

I Acknowledge the $500 deposit goes towards the purchase of the extension hair, and that once I make an appointment the hair is purchased. This means the $500 deposit goes towards both the product I purchase and the installation of said product.

I acknowledge that the $500 deposit is only a percentage of the total cost of the extensions and installation. This $500 deposit will likely be between 20%-60% of the total cost.

I understand the extensions are only my property once installation has taken place. Thereafter, the extensions are owned by me.

I agree not to have any services, adjustments, or removal outside of Studio J Salon.

I agree to book an appointment 8-10 weeks for removal, color retouch and re-installation of extensions. If for any reason I cannot commit to the upkeep of the service, the extensions will need to be removed by a certified stylist from Studio J Salon. The hair remains my property after removal. The hair may be reinstalled at a later date with the discretion of the stylist.

I understand that if I decide to cancel my appointment before installation that the hair is not my property.

I understand that any cancellations or changes to my appointment must be made at a minimum of 72 hours prior to my scheduled appointment. This includes appointments for move ups. A credit card will be required to stay on file before any appointments can be confirmed.

I agree that if I cancel within 48 hours of my scheduled appointment that I will be charged 50% of the total cost. This applies to installation, move up, removal, and re-installation appointments.

I agree that extension removal and reinstall must be maintained every 8-10 weeks, and will rebook accordingly.

I understand that coloring of the hair extensions may be necessary for the initial installation and as needed every 8-10 weeks during the reinstall service to maintain the desired look.

I acknowledge hair color matching and length were discussed during consultation.

I acknowledge that hair care was discussed in detail with stylist and that I was provided a copy of the hair care card.

*Please note, hair care is vital to ensure optimal results.

per my in-salon consultation and hair care plan understand and wish to proceed with the hair extension service.

I certify that the information that I supplied to my stylist during the consultation appointment acknowledgement and the hair care plan are correct. I agree to indemnify and hold harmless Studio J Salon free from loss, damage, claims and/or actions of any type that I suffer as a result of my failure to provide them with accurate and complete information prior to this agreement.

I agree that I will not hold Studio J Salon liable in any way for any loss, damage or injury suffered by me as a consequence of my use/misuse of the hair extensions.

I will indemnify and hold harmless Studio J Salon free from all loss, damage, claims of action of any kind that incur or that are claimed or brought against them by any party as a consequence of my own use of the hair extensions.
