A Licensed Personal Assistance Services agency must ensure that staff is screened at the beginning of each workday and that a client and the client’s household members are screened before a home visit. A PAS agency must also document that each screening occurred. This follows emergency rule §558.408 (Emergency Rule for HCSSA Response to COVID-19) and is to prevent the spread of COVID-19.

You must answer “NO” to all the questions in this questionnaire in order to comply with license regulation. If you answer “YES” to any of the questions, CALL the office (682) 276-1715. Note: This must be a NEW symptom experience.
If you experience any symptoms or answer “YES” to any of these questions, you must immediately contact your health care professional for recommended next steps AND notify the supervisor for the safety of the employee. All yes's must be reported to the local health department and we must determine if the visit is essential or can be postponed.
