Please answer all questions that pertain to you. If you have suffered any monetary losses, you must attach documentation (bills, checks, receipts, or estimates) for verification as the court will require this information when considering restitution. This must be done despite any information you may have given law enforcement. This document may be amended as changes occur and there will be other opportunities to voice your concerns.
