    Please complete this online enrollment form, and submit the following documents to the main office.

    • Original birth certificate (with raised seal), student Passport, or Visa
    • Official immunization record
    • Certified copies of court orders or placement papers, if applicable (i.e., Appointment of Legal Guardianship, Divorce Decree, etc.)
    • Parent/Legal Guardian driver's license, passport, or Visa
    • Proof of residency (two different utility bills or lease/rental agreement.)

    This enrollment form will be printed when you submit the documents listed above and you will be asked to review and sign this enrollment form.

  • Covert Public Schools collects information regarding the language background of each of its students. The information is used to determine who should be provided bilingual and/or English as a Second Language instruction under Title VI of the Civil Rights Act of 1964. Please answer the following questions concerning your child:
