I understand that only one mobile hot spot (if requesting use of them) is allowed per residence. I understand that violating this policy will result in the loss of the privilege to borrow a hot spot.*

I understand that the amount of time my residence can have a hotspot is 14 days and that my residence will not be allowed to check out another hot spot for 14 days from the return date (if turned in on time or before the due date) of the hotspot. Note: I may be able to renew the hotspot if it is determined by library staff that there are enough hotspots for others to check out.*

I will not place the Electronic Device in any of the book drops, but will return the Electronic Device and all accessories to the desk at the Middleton Public Library at 307 Cornell St. If I place the Electronic Device in a book drop, I will be subject to financial penalties and will not be able to check out an Electronic Device in the future.*

I will pay a late fee of $5.00 per day if I fail to return this Electronic Device to the inside front desk by the date due.*

I agree to pay all costs associated with damage to, loss of, or theft of the Electronic Device and accessories while it is checked out to me.*

I acknowledge that failure to pay any amount owing will be considered an outstanding debt to the Middleton Public Library and will be added to my library record.*

I understand the Library may use any appropriate means to collect the device (s) and amounts owed to the Library due to failure to return it by the Due Date. I understand that the device will be deactivated when it's 1 day overdue.*

I understand that there are no safeguards on accessible content and patrons are responsible for monitoring use by minors. The Library is not responsible for any liability, damages or expenses resulting from use or misuse of device, connection of the device to other electronic devices, or data loss resulting from use of device. Any use of the device for illegal purposes, unauthorized copy of copyright-protected material in any format, or transmission of threatening, harassing, defamatory or obscene materials is strictly prohibited.*
