By signing up submitting online form parent(s) or guardian(s) and Participant(s) have read, understand, and agree:

• We expect 100% positive Best Attitude, Greatest Effort, Laser Focus, Maximum Effort from coach(s), participant(s), trainer(s), parents(s), guardian(s), guest(s), family(s), friend(s), official(s), vendor(s), and fan(s), etc.
• We promote a Colorado Lightning “Family” attitude and faithfulness through high school and beyond
• We agree there is a great time commitment to Colorado Lightning Club, team, and teammates 100% attendance to every event is expectation
• We understand that this is a competitive basketball club and is NOT a recreational basketball program with NO minimum playing time guarantees or expectations
• We believe basketball comes first after church and major family events
• We acknowledge ALL events (training, practices, camps, team builders, etc.) are mandatory unless specified as optional. 100% attendance is the expectation to avoid falling behind the other players. Missing events will affect playing time
• We understand videos, pictures, other players likeness can be used for any purpose without any compensation or acknowlegment
• Coaches may organize and invite participant(s) to additional events during session. Every effort should be made to have 100% attendance to these events as well. There may be additional fees for these events (example: building entry to Nuggets game, training fee for special guest, event food or ticket, etc.)
• We do not tolerate negativity from coaches, players, families, or fans. No cursing, negative speech, bad attitudes, disrespect, bullying, hazing, fighting, violence, undermining coaches etc. will be allowed. First issue coach will talk to participants(s) individually instructing in proper expectations. Second issue coach will talk to participant(s) instructing in proper expectations informing parent(s) or guardian(s) and club president. Third issue we will have a meeting with participant(s), parent(s) or guardian(s), coach(s), and club president where written and signed action plan will be implemented. Colorado Lightning retains the right for any reason at any time to suspend and or expel participant (s) from any event(s) for andy reason(s)
• NO REFUNDS will ever be given for any reason
• We the parent(s) or guardian(s) of a candidate for a position on Colorado Lightning Basketball Club hereby give participant(s) approval to participate in any and all activities Colorado Lightning or any affiliated activities
• We the parent(s) or guardian(s) understand insurance (medical, liability, other, etc.) will NOT be provided by the Colorado Lighting Basketball Club or its affiliates. We the parent(s) or guardian(s) assume all risks and hazards incidental to such participation including  Colorado Lightning Basketball Club transportation to and from activities ant and all risks associated to covid-19 and do herby waive, absolve, indemnify and agree to hold harmless the entire Colorado Lightning Organization, coaches, participants, affiliates, vendors, and persons transporting participant(s) other individual(s)
• We will provide, immediately after accepting invitation to join Colorado Lightning Basketball Club, via email to ColoradoLightning@gmail.com (with player / participant name in email subject line) or provide / give a photo copy of certified birth certificate and photo copy of picture ID or school ID which shows current player grade level and full name. If coming to a tryout please have a copies to give at that time. All photo copies will not be returned to player and no photocopies can be made at event. A photo copy of report card which lists participant(s) full name and current grade level or photo copy signed letter on school letterhead with lists participant(s) full name and current grade level along with other picture ID with participant(s) full name can be substituted for school ID  
• The philosophy of our program is to give participants in Colorado, nationwide, and internationally an opportunity to play basketball at an elite level while mastering fundamentals preparing them to play at the next level(s). We Believe that Basketball is a platform for something greater and for us that something Greater is Jesus Christ.

Informed Consent and General Release Youth Athletics participant(s)

NOTE: THIS FORM INCLUDES A RELEASE OF LIABILITY.

• Participation in youth sports events, athletics, and activities can be dangerous. Colorado Lightning requires that all participant(s) and their adult parent(s) or guardian(s) assume all risks all by fully agreeing this general release and this entire online registration form as a whole
• For and in consideration of my child being permitted to participate in Colorado Lightning events, athletics, and activities I hereby voluntarily release, discharge, waive and relinquish any and all claims or actions for damages for personal injury, permanent disability, death, or property damage which I as parent(s) or guardian(s) or my participant(s) may have, or which may here after accrue to us, as a result of my participation Colorado Lightning events, athletics, and activities or for any other reason. This release is intended to discharge, in advance, Colorado Lightning, it’s officers, employees, vendors, affiliates, agents, and the owners and maintainers of any facility used for events or activities from any and all liability arising out of or connected in any way with my players(s) / participant(s) participation in any event, even though that liability may arise out of negligence or carelessness on the part of Colorado Lightning, it’s officers, employees, vendors, affiliates, agents, and the owners and maintainers of any facility used for events or activities. 
• I further understand that serious accidents or loss of personal possessions occasionally occur during youth events and activities, and that participant(s) occasionally sustain serious personal injuries, death or property damage as a consequence thereof. Knowing the risks, I have voluntarily applied for my participant(s) to participate in all Colorado Lightning events and activities and thereby agree to assume ALL risks to release and hold harmless Colorado Lightning, it’s officers, employees, vendors, affiliates, agents, and the owners and maintainers of any facility used for events  or activities, who (through negligence or carelessness) might otherwise be liable to parent(s) or guardian(s) or to my participant(s) or our heir(s) / assign(s) for damages.
• I further understand and agree that this release, discharge, waiver, and assumption of ALL risk is to be binding on parent(s) or guardian(s) or to my participant(s), heir(s) / assign(s), executors, and administrators.
• I further agree to indemnify and to hold harmless Colorado Lightning, it’s officers, employees, vendors, affiliates, agents, and the owners and maintainers of any facility used for events or activities for any loss, liability damage, cost or expense which may incur as a result of any injury or property damage parent(s) or guardian(s) or to my participant(s) may sustain while participating in events or activities.
• I agree to comply with the program’s stated and customary terms and conditions above and known for participation according to the Colorado Lightning. If I observe any significant change with regards to my participant(s) willingness or readiness for participation in the program, I will remove my child from the program immediately. No refunds will be given for any reason.
• I agree due to covid-19 no parents, guardians, family members, friends, or guests will be permitted to enter the location of team practices, workouts, meetings, or when gather for "team builder" social interactions. Some game or event facilities have their own rules you agree to follow their specific facility rules which may for example: only coaches and players will be allowed in event facilities or team must remain together in designated areas for duration of event. Dropping off or picking up participants remaining in vehicles in the parking lots allowing partcipant to walk to you safely making your detailed safe arrangements with your participant(s) ahead of time. The parent(s) or guardian(s) and the participant(s) have no reason to believe that the participant(s) has covid-19. The participant(s) does not have any of the following symptoms: fever, cough, shortness of breath, fatigue, muscle pain, chills, headache, diarrhea, nausea, or sore throat. Within the past 14 days the participant(s) has not had close contact with a person having or
  suspected of having covid-19. Within the past 14 days, neither the participant(s) nor anyone in the participant(s)’s household has
  traveled to any country, state, or city with widespread covid-19 diagnoses. Keep participant(s) away from team events if participants or household member(s) are confirmed or think they have contracted covid-19 or any other infectious condition.
• I agree as a parent(s) or guardian(s) to immediately contact coach(s) if participant(s) going to miss any event at lease 2 hours before start time of event or sooner if at all possible before start time of event.

I have read this Informed Consent and General Release Youth Athletics participant(s), fully understanding and agreeing its terms, the entire form, and that is binding indefinitely, that I give up substantial rights by submitting this entire form online digitally signing it and voluntarily do so. I understand this form may be updated or revised at anytime without notice it is parent(s) or guardian(s) sole reponsibility to review as often as deemed necessary however eSignature below is still binding even after future updates or revisions.
