If I have diabetes, epilepsy, hepatitis, hemophilia, HIV-AIDS, or any other communicable disease, heart condition or take medicine which thins the blood I have consulted and gained the approval from my doctor prior to having a tattoo applied and I have advised my tattooist. I do not have any other medical or skin conditions such as but not limited to: acne, scarring (Keloid), Eczema, psoriasis, rash, infection, lesion, freckles, moles or sunburn in the area to be tattooed that may interfere with the application or healing of the tattoo. If I have any type of infection, rash, or lesion anywhere on my body, I will advise my tattooist. I am not the recipient of an organ or bone marrow transplant or, if I am, I have taken the prescribed preventive regimen of anti-biotics that is required by my doctor in advance of any invasive procedure such as tattooing or piercing. I am not pregnant or nursing. I do not have a mental impairment that may affect my judgment in getting the tattoo.
I am not under the influence of alcohol or drugs, and I am voluntarily submitting to be tattooed by the Artist without duress or coercion.
A tattoo is a permanent change to my appearance and can only be removed by laser or surgical means, which can be disfiguring and/or costly and which in all likelihood will not result in the restoration of my skin to its exact appearance before being tattooed.
I acknowledge that I have been given adequate opportunity to read and understand this document, that it was not presented to me at the last minute, and I understand that I am signing a legal contract waiving certain rights to recover against the Artist and the Tattoo Studio.
The skin on the hand and foot is very different than the rest of the skin of the body, it is unstable. As such a tattoo on the hand and foot has issues and consequences that are different from tattoos on other parts of the body. Such as: the tattoo will spread more and faster than other body locations which will cause smudging and/or blurring and/or colour may bleed outside the lines and settle where they are not supposed to be, the body may not accept tattoos on the hand and foot especially the palms of the hand and soles of the foot, often a ring tattooed on a finger will not take well on the palm side or the sides leaving a patchy mess, and there is greater chance of infection associated with a hand or foot tattoo. There is no GUARANTEE on tattoos on the hand or foot. I understand the full risk of hand and foot tattoos and I agree that the work may not turn out as I expected and any and all touch-ups or repair work will be at the expense of the client.
