[Call the patient]. Hello, my name is {nameOf} and I am calling from Pitkin County Public Health. Am I speaking with {patientName}? Before moving forward, can you please verify your date of birth to ensure I am speaking with the right person ({patientDate})? Thank you for verifying that. I am calling because we have received results from a recent medical visit you had regarding COVID-19.
IF THEY HAVE NOT BEEN INFORMED OF THEIR RESULTS
Okay, I'm sorry you have not yet been informed. Your results indicate that you have tested positive for COVID-19, the novel coronavirus. This means that you have a current infection of COVID-19. I know this can be scary or concerning, so before we move forward, do you have any questions I can answer about your test results at this time? (If yes, answer questions; if no, move on).
Okay, thank you. So, to reiterate, your results indicate that you have tested positive for COVID-19, the novel coronavirus. When someone tests positive, we, at the health department, call to discuss these results with you to 1) find out if you have any questions, 2) verify some information, and 3) discuss with you who were around while infectious. For your awareness, I am required by law to keep all information you share with me confidential. So, before, we move on, do you have any questions for me at this time?
So, because you have tested positive for COVID-19, we will need you to continue to isolate yourself until the following criteria have been met:1. You must not have had a fever for at least 3 days; AND2. Any respiratory symptoms you are having must have improved; AND3. It must be at least 10 days since you developed your first symptom.
As a reminder, isolation means you stay home to avoid transmitting this disease to others. Walking outside with a mask on and avoiding all contact with others is permissible, but you should not grocery shop, travel in a car or on public transportation, or go to work. We will provide you with an isolation letter that you can share with your employer if needed. This means that we will also need to ask your close contacts to quarantine to ensure they don't transmit this virus to others as well.
Given that isolation can be burdensome, the county may be able to provide financial and food assistance during this time if needed. With that said, would you like for us to make a referral to the county on your behalf to help you while you isolate?
Please complete the Google Form for the AFN Referral, then return here to complete the interview.

I will be following up with you over the next several days to make sure you're doing alright. Do you have any additional questions?
