Welcome to my Ayurvedic Health Counseling and Body Therapy practice. I am so honored that you have begun investing in your health with me! This is a journey, a process that we will go through together. I am very passionate about increasing the quality of health and life for myself and others and sharing the wisdom of Ayurveda.
As you know, I am a practitioner of Ayurveda. I am not a licensed physician, nor are Ayurvedic services licensed by the state. The idea behind Ayurveda is to address your needs on an individual basis to restore balance in all aspects of your being.
Ayurveda is a natural healing system originating in India that has been practiced for thousands of years. Although Ayurveda has some general rules and recommendations for everyone, it is absolutely individualized in use for both maintaining health and healing imbalance.
During the consult there will be a conversation that could pull from many parts of your life and history. There is absolutely no need to feel pressured to answer anything that you’re not ready to share. Just answer with a “pass” and we will continue on!
This consult is based on ayurvedic principles and not western medicine. This is NOT a replacement for your current doctors, medicines or treatments. Please continue to follow their care and prescriptions. You will not be diagnosed or treated. No medicine will be prescribed.
Everything I suggest to you will be completely natural. Ayurveda follows the laws of nature, so by altering your lifestyle and adjusting to the natural cycles, you will create an optimal environment for healing...and thriving!
You will receive a list of recommendations that may include the following:
• daily routine
• foods, spices, teas
• natural non-prescription plant-based herbs
• ayurvedic massage, body therapies, and cleansing/detox programs
• exercise
• yoga practices - postures, breath work, meditation, chanting
All forms, recommendations and conversations are kept completely confidential.

In order to use my services, State law requires that you acknowledge receipt of the information provided in this form and that you sign it and you may request a copy. I will keep the original in my records for at least three years.
My method of treatment is alternative or complementary to healing arts that are licensed by the State. If you ever have any concerns about the nature of your treatment, please feel free to discuss them with me.

I have read and understand the above disclosure about the treatments offered by Seva. I have discussed with Seva the nature of the services to be provided. I understand that Seva is not a licensed Physician. I consent to receive the services offered by Seva Ayurveda, and agree to be personally responsible for the fees of the services provided to me.
