Southern Union Assessment Center Test Candidate Rules Agreement

Please review and submit the following test rules. If you have questions, please ask the Assessmemt Center staff.

I will not take the following types of personal items into the testing room: cellular phones, hand-held computers/personal digital assistants (PDAs), and other electronic devices; pagers, watches, wallets, purses, hats, and other head coverings; bags, coats, jackets, hoodies, vests, and other outwear; books and notes, etc. (only if allowed). Studying is not allowed in the testing center. *

I will store these items in a secured area designated by the Assessment Center staff.Cellular phones, pagers, and other electronic devices must be turned off prior to placing them in the designated secure area. The Assessment Center is not responsible for lost, stolen, or misplaced personal items. *

The Assessment Center staff will check my ID and administer me the test at my assigned seat. I will sit in my assigned seat until I finish my test. I understand that eating, drinking, smoking, chewing gum, talking, or making noise that creates a disturbance for other candidates is prohibited during the test. *

The Assessment Center staff will monitor me continuously while I take the test. The session may be videotaped or otherwise recorded. *

If I experience problems that affect my ability to take the test, I will notify the Assessment Center staff immediately by raising my hand. I will not talk in the testing room. If I have other questions or concerns, I will raise my hand for assistance from the Assessment Center staff, making sure other candidates are not disturbed. The Assessment Center staff cannot answer questions related to the test content. If I have questions of this nature, I will contact my instructor. *

I understand that there are no scheduled breaks during testing. Unscheduled breaks, for any reason, may cancel the test. *

I will not remove copies of test questions and answers from the Assessment Center.I will not share or discuss with others the questions or answers I have seen on the test. *

After the test ends, the Assessment Center staff will receive all testing materials and deliver them to a designated secure place. *

Your Privacy:
Your test results will be sent to the instructor as directed from the request sheet. Your questions should be directed to your instructor.

Candidate Statement:
By signing below or providing a digital signature, I give the Assessment Center staff at Southern Union State Community College my explicit consent to retain and transmit my personal data and test responses to my instructor. I understand the information provided above and agree to follow the rules. If I do not follow the rules, or I am suspected of cheating or tampering with the computer, I will be subject to disciplinary action as stipulated in the Southern Union State Community College Handbook.
