*Please only fill this out if you have a scheduled appointment. This form is for new AND established patients!*
- We check in our appointments curbside for the comfort of our patients. Clients are always welcome in our facility for their pet's exam. Please remain in your car and call us to let us know you are here.
- For the comfort and safety of all of our patients, please do not wait by the entrance or crowd the sidewalk. Some of our patients are fearful or people/dog reactive.
- Please make sure cats are securely in a carrier and that dogs are secured on a leash when you enter the building. We kindly ask that prong or electronic collars be removed when you bring your dog into our facility. Flexi-leads should be locked.
  If you need a cat carrier, a collar, or a leash, please ask us!
- For clients wanting guidance on behavior, we have specific slots reserved for these consults weekly. Please understand that these discussions are outside the scope of a regular wellness exam or for clients that are establishing their pet with us.
- We welcome clients to take pictures and videos of their pets during an appointment, but if a staff member is in the shot, please check with them first, especially if you are posting on social media! Thank you :)
Curbside appointments will be provided at request if preferred.

PRESCRIPTION POLICY REMINDER:

Brooklyn Yard Veterinary Hospital WILL NOT process (phone, email, fax etc.) prescriptions to or from outside and online pharmacies (with the only exception being for medications that need to be compounded).

Clients who opt to obtain their pet's medication through these avenues will need to pick up a physical written prescription at our hospital.
