Consent:

I, the undersigned owner, or the owner's agent, of the pet named above certify that I am over 18 years of age and hereby authorize the veterinarian(s) at Hopewell Veterinary Hospital to perform the above service(s) and/or procedure(s).  I understand that should some unexpected life-saving emergency care be required and the hospital staff be unable to reach me, Hopewell Veterinary Hospital has my permission to provide such treatment and I agree to pay for such service. I have read and I fully understand the terms and conditions set forth above.
