I hereby authorize Scott County School District 2 (SCSD2) to have the amounts indicated above deducted semi-monthly (twice per month) from my paycheck for each future payroll until I request in writing (email) a change or to discontinue. The requested deduction will begin with the next payroll following submission if processing time allows but will start no later than the second payroll period after your submission is received. I understand that payroll deductions will continue until I contact the SCSD2 Business Office to cancel or change deductions.*
