Introduction and Purpose
We are a global and diverse team of over thirty enthusiastic, ambitious youth ambassadors and students from more than fifteen universities. We are passionate about combining tech with global health, social good, and equity in order to fight the pandemic. We are mentored and backed by experts and research groups from various prestigious academic institutions, firms, and companies.
Under the banner of Virufy, we are conducting a research survey to collect information related to the COVID-19 pandemic.
We would like to invite you to take part in our research survey for the development of an app for COVID-19 screening.
By collecting data through our research survey, we hope to develop an app with artificial intelligence (AI) technology that will use a smartphone (or other device) as a COVID precheck tool.
As part of the study, you will be asked to provide certain personal information, biometric and health information, and audio recordings. This is important because your audio recordings will be used for training machine learning models (central to AI technology).

If you agree to participate in our research, we will ask you to complete the attached online survey. The survey will involve questions about your current medical conditions, respiratory conditions, symptoms, smoking status, basic demographics, and COVID-19 testing results. Additionally, we will ask you to submit audio recordings. All the information that you provide us through this survey, we will call hereinafter the "information collected" or "requested information".
The survey should take less than five minutes to complete.
The requested information may be considered sensitive information (or special category data in the European Union). We will also request contact details (e.g. email and phone number) so that we may contact you by email and/or text messaging (e.g. SMS and WhatsApp) in relation to your COVID-19 infection and/or testing status, the cough or speech sample you have donated, and/or to remind you to donate subsequent samples.
This study involves collecting a cough and speech sample recording through your phone and therefore has a risk of spreading COVID-19 and other germs.
● To reduce this risk we ask you to record the cough alone and in a private space with no other persons wherever possible and in any event from at least 6 feet distance from other persons. You should also be sure to disinfect your smartphone and any affected or nearby surfaces after recording your cough.
● To reduce the risk of spread of germs we advise you to use your own phone to record the cough sample and to also wear a mask where appropriate.
As with all research, there is a chance that confidentiality could be compromised; however, we are taking data security precautions to minimize this risk.
Your study data will be handled as confidentially as possible. The biometric information and health data collected will include the following categories of information:
Calibration recording (e.g. speech)
Cough sample recording
Test results for COVID-19
Timeframe of COVID-19 tests taken
Current COVID-19 symptoms
Relevant current respiratory conditions
Relevant current medical conditions
Contact details (e.g. email and phone number)
We collect and process personal information and biometric and health information (deemed to be special category data under the GDPR) only where you have given your explicit consent via this Consent Form:
To conduct research and develop the App for smartphones and devices that uses artificial intelligence to screen for COVID-19;
To train machine learning models to analyze your audio recordings in order to better determine early signs of COVID-19;
To conduct research and develop future applications that use artificial intelligence to screen for other viruses and medical conditions;
To train machine learning models to analyze your audio recordings in order to better determine early signs of other viruses and medical conditions; and
To conduct demographic analysis of COVID cases and other research beneficial for the medical community.
The information collected will be kept secure using a database with restricted access.
We may distribute your personal information to partners for future research studies, in a form that may or may not identify you, without additional informed consent. We may also publish or present the results of this study containing your anonymous information. Your personal information may be released if required by the law.
We may share your anonymized personal information in the following ways:
As an open source dataset for use by researchers globally to accelerate development of similar technologies and/or to better arrive at a solution.
We may also share your non-anonymized personal information in the following ways:
With another legal entity, on a temporary or permanent basis, for the purposes of a joint venture, collaboration, financing, sale, merger, reorganization, change of legal form, dissolution or similar event. In the case of a merger or sale, your personal information will be permanently transferred to a successor company. If a change happens to our business, then the new owners may use your personal information in the same way as set out in this Statement.
Where needed to enforce our rights, protect our property or protect the rights, or safety of Virufy and others, or to support external auditing, compliance and corporate governance functions.
Public authorities where we are required by law to do so.
If required, in order to receive legal advice.
If results of this study are published or presented, results will be presented in such a way that you will not be personally identifiable.
We will store your biometric and health information in a form that permits us to identify you for no longer than is necessary for the purposes for which the personal information is processed. For the avoidance of doubt, biometric and health data (which may or may not be anonymized) may be retained long term, for future research studies into viruses and other medical conditions.
There is no direct benefit to you from taking part in this study today. We will not provide you with any tests or other results relating to your COVID-19 status at the time of submission. The information collected may not benefit you directly, but your participation may help others with COVID-19 risk in the future.
There are no costs to you for participating in the study, except for any Internet data charges you may incur (e.g. WiFi, data roaming and cellular costs).
You will not be paid or otherwise compensated for taking part in this research.
Participation in this research is completely voluntary. You are free to decline to take part in the project. You can decline to answer any questions and are free to stop taking part in the project at any time. Whether or not you choose to participate, to answer any particular question, or continue participating in the project, there will be no penalty to you or loss of benefits to which you are otherwise entitled.
However, if for any reason later, you would like the information collected to be removed from our database, you may contact email@example.com.
