Please read carefully and sign below in order to participate in the Studio V Dance Program. I understand and agree that participating in any Studio V Dance class, camp, or workshop, is a possibility of physical injury and, in rare circumstances, death.

I agree to release and hold harmless Studio V Dance, including its teachers, dancers, and staff members, from any cause of action, claims, or demands now and in the future. I will not hold Studio V Dance liable for any personal injury or any personal property damage, which may occur on the premises before, during, or after classes.

Furthermore, I understand that I should be aware of my and/or my child’s physical limitations and agree not to exceed them. I am signing this waiver for myself and/or my child, I certify that I am over 18 years of age or the parent or legal guardian and have the right to waive these rights.

Further, I authorize and agree that Studio V Dance may record the class instructors ONLY for purposes of record-keeping, marketing, and instructor/content assessment, and understand that Studio V Dance will never record my child’s image.

By signing the waiver you acknowledge that you have understood and agreed to the terms of the waiver. Please use the parental or legal guardian email address, or email address associated with a person of 18+ years of age and your child's name when filling out the below waiver.
