I First Name Last Name resident of unit Unit number*, would like to schedule a delivery of describe item* for the following date Date and time Time (Only allowed between 9am and 5pm, Monday-Friday; no Deliveries on weekends or on Holidays).

1. I agree to indemnify, hold harmless and defend West Ocean Association, the Board of Directors, Action Property Management and their employees, from any and all claims for damage, liability, loss of property, expense and costs in connection with the delivery (including, without limitation, attorney’s fees and costs of enforcing this indemnity).2. I am aware that a pre-inspection and post-inspection of the common areas will be performed in relation to the delivery, as to ensure that any pre-existing and/or new damage is documented accurately.3. I agree to pay for any damage to the common area due to the delivery activity.4. I understand that my delivery is not confirmed until I receive an email from management.5. When the delivery arrives, I agree to notify the front desk and agree to have a member of the management or maintenance team accompany the delivery and will not take them up to the unit myself.6. Deliveries must be kept under 30 minutes to avoid a Moving fee.
