This form is to be completed yearly by returning, active GNC volunteers to fulfill Child Protection and Workers Compliance Requirements.
*This form is only open for returning volunteers. If you have not attended a Child Protection workshop within the last 5 years, this form will be rejected. Please contact our local office at 704-871-2424 if you have questions.
