Consent: By signing this form, I certify the information I have reported on pages 1-2 of this intake and assessment form is accurate to the best of my knowledge. I am interested in pursuing a program of secondary or postsecondary education and need the services of the TRIO Educational Opportunity Center.

To have access to my high school and college information necessary to determine program eligibility, provide assistance and to verify enrollment in college; this includes my transcripts, test scores, student financial aid records, and enrollment status.

To release information from my participant file to state and federal agencies when required by law or regulations.

If accepted into the program, I give permission for the use of my name and/or photograph for editorial, promotional, recruitment, or educational purposes.

I grant EOC the right to take photographs and video of me/the student. I waive and release any rights to such photographs and video, and I agree that EOC may use such photographs and video, without any compensation, with or without my/the student's name, and for any lawful purpose, including publicity, illustration, advertising, and web content, in perpetuity.

This form is used by the TRIO Educational Opportunity Center (sponsored by the University of Northern Iowa and funded by the Department of Education) for the sole purpose of obtaining transcripts and pertinent school records to those students who apply for our program. The prospective student’s signature on this form grants the Educational Opportunity Center the right to request transcripts/records/standardized diagnostic or achievement test scores to institutions previously attended, solely for counseling purposes.
