Meadow Brook Intermediate School Residency Verification Submission
**This submission form is for Meadow Brook Intermediate students ONLY**
All Mustang Public Schools students will need an updated Proof of Residency submitted on their behalf prior to the beginning of school. This submission form will be open from July 27th to July 31st. Before you begin your submission, you will need to have electronic copies of your Proof of Residency and parent/guardian Photo ID. The Proof of Residency and Photo ID will be uploaded to this submission form from your computer or device. Acceptable proofs include: gas, water, electric bills, warranty deed statement, ad-valorem tax statement, mortgage statement, rental agreements, or construction contract (cable and phone bills are not accepted). Completion of the Residency Verification is necessary for students to receive their 20-21 schedule and/or homeroom teacher. If you have more than one student at Meadow Brook, you will need to submit a form for each student.
