  • [SCOR] Service Center Operations Report (Remote)


    The SCOR report is used by Field Managers and Training & Standards to review compliance oversight and identifying training opportunities. It's important to remember to answer each question accurately and honestly. Our goal is to ensure all our centers are in full compliance with state regulations and provide our customers with the best experience possible. 

    Unlike an in person SCOR visit this will take less time but will also require some additional work:

    • Contractors and technicians will need to submit photos to you of various things in the service center. These include: Calibration Equipment, Inventory Storage, and Workstation Setup. Example photos can be found in the appropriate sections below.
    • SCOR administrators will need to review recently completed paperwork for each service center. 2 installs, 3 monitors, and 2 violation resets will need to be reviewed prior to submitting the SCOR. Client ID numbers and dates of service must be entered for each.

    This section will require the SCOR administrator to randomly lookup recently serviced clients from the service center being evaluated.

    At least 2 install appointment, 3 monitor appointments, and 2 violation reset appointments must be reviewed for accuracy. Client IDs and the appointment dates must be entered below.
