Before completing this form, note the following requirements for all Dynasty Elite Members:

This is a last call audition for dancers with previous dance training only.

Members are required to attend all Summer rehearsals (Friday's and Saturday's included)

Location: 6003 Marlboro Pike District Heights, MD 20747

Notes
- Dancers will be adjudicated based on competitive and classical dance training and vocabulary in varius dance styles that include Ballet, Jazz, Lyrical, Acrobatics, Hip-Hop, Tap and Contemporary dance styles.
- Dancers must wear ALL black dance attire and bring all dance shoes.
- Dancers must arrive on time with hair neatly off of the face.
- Mandatory parent meeting directly after auditions.
- Audition Fee: $0.00
- Dancers must be accompanied by an adult.
Audition Date Sunday August 6th

Ages 4-10 4pm-5pm (Tiny, & Mini divisions)

Ages 11-17 5:30pm-6:30pm (Jr. Teen, & Sr., divisions)

Informed Consent and Acknowledgement

I hereby give my approval for my child’s participation in any and all activities prepared by Neema Dance Collective during the selected audition. In exchange for the acceptance of said child’s candidacy by Neema Dance Collective ., I assume all risk and hazards incidental to the conduct of the activities, and release, absolve and hold harmless Neema Dance Collective, LLC and all its respective officers, agents, and representatives from any and all liability for injuries and or diseases to said child arising out of traveling to, participating in, or returning from selected camp sessions.

In case of injury to said child, I hereby waive all claims against Neema Dance Collective including all coaches and affiliates, all participants, sponsoring agencies, advertisers, and, if applicable, owners and lessors of premises used to conduct the event. There is a risk of being injured that is inherent in all sports activities, including basketball. Some of these injuries include, but are not limited to, the risk of fractures, paralysis, or death.
