All participants in BONNER COMMUNITY HOSPICE choose to assume ethical responsibility to clients, families, ourselves and each other.
Clients/families share circumstances and personal matters in good faith, expecting confidentiality. I am obligated by law and ethics to honor the following pledge:
1. Information concerning the client/family may be discussed for clinical purposes only.
2. No identifying details about client/family (such as address, age, sex, disease) may be revealed except within the Hospice Team.
3. Case records and charts are confidential for clinical and statistical purposes within Hospice. No information will be released to other agencies except by authorized physicians and nurses directly involved with the client. Other agencies must obtain a “Release of Information” from client or guardian to gain access to Hospice records.
4. If a client’s case is made public through media or other sources, it does not affect my confidentiality oath and I will not comment on any information.
5. Descriptive details of a case, even without identifying client/family directly, could be a breach of confidentiality.
6. A breach of confidentiality could be grounds for termination of my participation with the programs of BONNER COMMUNITY HOSPICE.
