Kingsview Village Seventh-day Adventist Church Parental Consent Form for Children on Cyberspace
Please read the following carefully and fill in all information accurately.
Parental Permission Form for Minor’s Online Participation in iWonder Vacation Bible School Program, August 3-6, 2021.
The Kingsview Village Seventh-day Adventist Church values the safety of ALL, especially minors, with the highest regard. Our church will continue to provide services to participants, while following all safety protocols and adhering to governmental security and safety bylaws. As such, the Kingsview Village Seventh-day Adventist Church will ensure the following while the iWonder Vacation Bible School Program is in session:
-There will always be at least two (2) adults present on the Zoom platform while the program is in session.
-It is mandatory for all volunteers to have a valid vulnerable sector check.
-Volunteers are NOT permitted to communicate with children privately or one-on-one through any means, including SMS texting, online messaging/social media, text messaging through Zoom chat, email, or any similar media. Parents are also NOT permitted to communicate with minors through any means, including those aforementioned. Exceptions will be made only for parents communicating with their own children. Violation of these protocols will leave volunteers and/or parents subject to disciplinary action, including disbandment from the iWonder Vacation Bible School program.
-We kindly ask that while the iWonder Vacation Bible School program is in session that NO photos, video, or other visual media, as well as web links be shared by parents, children or unauthorized volunteers via the Zoom chat or email to other registered participants or volunteers. There are great risks involved with such activity, including the spreading of viruses and malware and breach of privacy. Once a file, media, or document is shared online, it remains online forever.
-In order to maintain participant privacy, each daily session of the iWonder Vacation Bible School will NOT be recorded. Volunteers, parents, and participants are not permitted to record the daily sessions of the iWonder Vacation Bible School with their own personal devices by any means.
Further, I/we understand agreeing to allow my child to participate in the online activities identified above involves certain risks such as exposure to bullying, inappropriate content, misappropriation of personal information and other risks associated with online activity. In addition, the Kingsview Village Seventh-day Adventist Church cannot guarantee that participation in this online activity will not expose your hardware to viruses, and other malicious software or code-based tools, though highest preventative measures to minimize all risks and exposure will be taken.
I/we still wish to proceed with the activities described herein and assume all risk and understanding of the risks involved.
I understand that supervision of what my child accesses online and the information they share are my responsibility. I agree to fully supervise all activities my child participates in.
