Please provide the following information in the event a parent cannot be reached. If we need to use this information we will also attempt to call the emergency contacts listed on this form.
LifePoint Youth is continuing to follow the precautions we have set in place for as long as they are recommended. These include:• Asking everyone to wear a face covering while at church if not exempted as per Pima County regulations• Screening temperatures at the door• Encouraging hand washing, cough-covering, and social distancing• Providing soap and hand sanitizer• Sanitizing / wiping down high-touch areas after every gathering• Using visual aids to demonstrate what 6 feet of distance looks like• Keeping items that multiple people touch to a minimum• Spacing out seating as much as possible within the rooms• Providing only snacks and drinks that are individually prepackaged except as otherwise noted• Keeping all people (including parents) who are not part of the class out of the youth area as part of redeveloped check-in and check-out procedures to reduce the number of people each teen comes in contact with• Encouraging people who are sick or at-risk to stay home and utilize online opportunities to connect
We are SO thankful that we are finally able to be in the same room TOGETHER! When you and your teen follow these guidelines, you not only protect your family but the rest of the church as well. Our team will make every reasonable effort to keep the environment safe and sanitary, but we cannot guarantee the total absence of any virus. By allowing your child/teenager to attend LifePoint Youth activities you acknowledge that despite all the precautions we are taking, it is possible that s/he may get sick. Thank you for understanding and for entrusting your family to LifePoint Youth!
