I have received and read a copy of First Bible Baptist Church’s Policy/Procedure Guide and agree to the following:
- to become familiar with all information contained therein.
- to comply with the information contained herein.
- to prevent the sexual, physical and emotional abuse and neglect of children in all ministry related functions
- to report any instances of such abuse or neglect of which I become aware
- to report any violations of the ministry’s child protection policy

These guidelines have been designed to guide and assist you when working with children and youth. The information establishes general practices and guidelines. First Bible Baptist Church reserves the right to make changes in the content or application of this program and to implement those changes with or without notice.

NOTICE – BACKGROUND INVESTIGATION

In connection with your employment/volunteering with First Bible Baptist Church, notice is hereby given that a background check for employment/volunteering purposes will be conducted. The reports may also contain information about you relating to your criminal history, or other background checks.

You have the right, upon written request made within a reasonable time after the receipt of this notice, to request disclosure of the nature and scope of any investigative consumer report prepared by contacting the First Bible Baptist Church and Protect My Ministry 14499 N. Dale Mabry Hwy., Suite 201 South, Tampa, FL 33618; Phone: 1-800-319-5581. For information about Protect My Ministry’s privacy practices, see www.protectmyministry.com. The scope of this notice and below authorization is not limited to the present and, if you are hired or accepted to volunteer, will continue throughout the course of your employment/volunteering and allow the First Bible Baptist to conduct future screenings for retention, promotion or reassignment, as permitted by law and unless revoked by you in writing.
