This form is for those who wish to transition from the on-campus classroom to virtual class attendance. This is NOT the same as taking a class that is fully online. This form is specifically for those who need an alternate mode of delivery due to COVID-19. Examples include, but are not limited to a compromised immune system of yourself, or that of someone in your home, the inability to wear masks because of medical concerns, the need to home-school children, or the need to quarantine for an indefinite period of time. This form is for those who want to transition to virtual course attendance for all of your classes for the ENTIRE semester, or the remainder of the semester if you are submitting after classes have begun.
If you anticipate missing one or two classes, please communicate with your professor.
This application goes automatically to the Dean of the School of Counseling or the Assistant Dean of the School of Ministry. Once the application is approved, the registrar will be notified, who will then notify the student's professors and the student's advisors.
