This information helps us take the necessary steps to protect the safety and well-being of our students.

To the best of my knowledge the information provided above is true and accurate. I understand that my failure to truthfully and accurately state whether I have been convicted of a crime may constitute grounds for denial at this program. I hereby authorize the Exposure Project to conduct work history, personal reference, police record inquiries including the National Sex Offenders Registry to determine my acceptability for volunteer work with children. I also understand that as a volunteer, I will not be alone with a student(s) at any time.
