• Emergency-Room Patient Intake

This is an experiment for my masters final project. Please read the following instructions before proceeding.
  • Instructions -

For the following form, you will be asked a series of questions, which resemble the medical intake during a visit to a hospital emergency-room.
  • To answer the follwing questions in a way as authentically as possible, as if you are currently visiting the hospital for a health problem, try to remember a visit to a hospital you had in the past. Try to remeber the physical and mental state you were in while visiting - it's important for getting the most authentic results.

In case there is no actual past visit you can think of for sharing, you could also try and imagine a fake visit. To give it a complete background, try to think of the person you are simulating and his whole character. It might help think of a person you know well - family memeber, coworker, friend or even a TV character.

Last remark! The answers to these qestions must be given in English, and as voice recorded answers. The answer field is actually a recording field, so you will probably have to give a local permission on your device to allow the recording of your voice. I hope you trust me on this one :]

Make sure your answers are as long as mentioned. For the succes of the experiment, I need your extended answers, so please, don't hold any thing back, and share as much information and descriptions as possible, with any relevant facts you can think of. Even zodiac signs are relevant ;)

Thank you for reading this far.
