    ***Please note that this application will qualify you only as a foster. Being approved to foster does not automatically mean you will be approved to adopt. If you are planning to adopt at some stage, and hope to meet your dog by fostering, please request the more detailed adoption application.***  We are delighted that you are interested in fostering for us, and are grateful for your offer of help. We view our relationship with our fosters as a partnership, and we will work with you to make it succeed.Although the rewards are huge, fostering can be challenging and hard work. Sometimes it’s heart wrenching, usually it’s messy, and it is rarely convenient. This application is intended to ensure that you know what you are letting yourself in for, and ready to make the commitment. At the same time, it will inform us about your strengths, limitations, and areas where you may need extra support. Our goal is to place dogs in foster homes where they will fit with the foster’s lifestyle, capacity and expectations.We will conduct a background check on each adult living in the home.Please answer all the questions in as much detail as possible. We will call soon to chat with you further and to set up a home visit.

    We want to ensure that a dog we place with you will be able to cohabit peacefully with other animals in your home.

    Please list the pets that you currently have in your home. Please provide the pet’s name, species (dog/cat/bird/etc.), breed (if a dog), age, dominant or submissive (if a dog), sex, whether it is sterilized, and whether it is used to other dogs.

  • BEFORE hitting submit please read these important terms & conditions:

    Mikey's Chance Canine Rescue will be represented by the abbreviation MCCR in the following.

     

    • Your foster dog may not be house-trained and puppies can be messy. You understand he/she may have accidents in your home.

    • Like many dogs, your foster may chew on furniture, clothing, or other objects.  You are comfortable working with this behavior.  The foster home is responsible for any damage.

    • You agree to keep your foster dog on a leash, enclosed area, or inside your home at all times.  

    • You will NOT take your foster animal to a veterinarian or administer medications unless directed to do so by the Directors of MCCR.  MCCR will not reimburse foster volunteers for any unapproved veterinary expenses.

    • MCCR may need to contact or visit you to discuss the foster dog.  You agree to be entirely honest and forthright in regard to your foster pet’s condition, be it positive or negative.

    • There is some risk to your own animals, especially if your foster animals are not kept separate.  You understand that MCCR is not responsible for your own pet’s medical treatment.

    • MCCR is the legal guardian of your foster dog.  You understand MCCR has the final authority in regard to the animal’s adoption, treatment or disposition.
    • You will not leave your foster dog in the care of another person, including potential adopters, without approval.
