YOU MUST HAVE A ZOOM ACCOUNT TO PARTICIPATE IN THIS CLASS.

Please make sure you enter the email address that you use for your ZOOM account and check to make sure you receive an email confirmation. The email will contain important information about the class you have scheduled. If you do not receive a confirmation registration email from JotForm within 24 hours, or think you have done it incorrectly, please call our office at (518) 459-2040 to confirm your reservation.

* If you do not provide us with the correct mailing address, including apartment number, we are not responsible if you do not receive your completion certificate within 7-10 business days from the day that you took the class. If you do not provide the correct mailing address, there may be a $20 replacement fee for a new certificate.

If you can not attend a class you must cancel that class by calling the office prior to rescheduling. You will receive the link for the class the day of the class within an hour of the start time.
