2. Arrival
Please arrive on time for your appointments. If you arrive late, you'll only be able to receive the remaining time previously scheduled for you. You will be charged the full rate for your session.
3. Payment
Payment is required at the time of the appointment/invoice. If HICAPS services are unavailable, it is your responsibility to claim rebates from Medicare or your Private Health fund. Payment arrangements can be made at the discretion of the Practice Manager or Directors. Non-compliance with payment arrangements may result in the arrangement being cancelled.
If you have funding including (but not limited to) DSS, NDIS or Medicare it is your responsibility to ensure your funding covers the therapy provided. By signing this document, you acknowledge that if funding is inadequate to cover the therapy or is no longer available, you will be liable for all outstanding amounts with Village Therapy.
Non-payment of invoices will result in future appointments being cancelled. Village Therapy reserves the right to recover the administrative costs of debt collection if this becomes necessary. Invoices may be referred for debt collection at any time.
