Working Dogs for Conservation is taking reasonable precautions to try to prevent the spread of the Coronavirus, including, for example, limiting employee visits to clients, requiring telework where possible, encouraging frequent handwashing, regularly wiping down equipment, and limiting face-to-face gatherings and meetings. Due to the nature of our business, however, we cannot truly practice “social distancing” 100% of the time.
As a volunteer, we are relying on your assistance in our efforts to prevent the spread of the COVID-19 virus. According to the Center for Disease Control (the “CDC”), symptoms of COVID-19 (“COVID Symptoms”) include, but are not limited to, cough or shortness of breath (symptoms that may appear 2-14 days after exposure) AND/OR the presence of at least two of the following symptoms: fever, chills, repeated shaking with chills, muscle pain, headache, sore
throat and new loss of taste or smell (https://www.cdc.gov/coronavirus2019-ncov/symptoms-testing/symptoms.html). If you or anyone in your household or to your knowledge anyone else with whom you have had close physical contact has experienced COVID Symptoms within fourteen days before any anticipated
future personal contact with any of our employees or agents, please notify us immediately. This will likely mean that for any future meetings in the short term, all of our employees and agents will avoid any personal contact with you, even though social distancing could be practiced.
If you have been free of COVID Symptoms, our employees are prepared to meet with you in person if alternative means of interaction and communication are impractical, but we require that our employees and you practice social distancing at all times as required by CDC guidelines.
Your signature on this form, verifies that you are aware of the potential risk of contracting COVID-19 during interactions with our business, accept and assume this risk, and have chosen to proceed with interactions with our employees and agents. Your signature also verifies that neither you nor anyone in your household has shown any COVID-19 Symptoms within the last fourteen days, or if such symptoms have been present, the illness has been diagnosed by a
licensed healthcare provider as something other than COVID-19.
