Security: Every coaching room is under video/audio surveillance at all times. This will allow us to monitor program activity implementation and behavior in all rooms.
Declined Credit Card/NSF Check Policy: There will be a $35 fee for any checks that are returned for insufficient funds. If the account is not brought current within 48 hours there will be a halt to services until account is current. If you are paying with credit card and that payment is declined you have 48 hours after being notified to bring your account current.
Student behavior: Students will be required to put forth their best effort in all activities assigned to them in their coaching sessions. If a child becomes a distraction or refuses to comply with their assigned activities, they will be removed from their coaching session and their parent will be contacted. The following steps will be exercised: 1. parent consult including a discussion with the child. 2. trial period to allow the child to comply fully with the activities.
If the behavior continues after intervention is attempted and the child refuses to comply, Brain Sprints reserves the right to cancel coaching hours and excuse the participant from the program with no refund given.
Food and Beverage: Please send a water bottle with your child when they come for coaching sessions. Food and beverages will not be allowed in the coaching rooms other than water. If there is a medical need for a snack during the time that a child is in their coaching sessions please let us know and send what they need with them. Allowances will be made for medical needs.
No Show Policy: Coaching hours are set up for you on a monthly schedule. Please let us know as soon as possible if your child will not be able to attend their scheduled coaching session.
If you are planning on taking a vacation during regularly scheduled coaching weeks there will be one week available per year for make-up. No other make ups will be given for missed time.
Brain Sprints has built in six weeks of break time throughout the year to allow for travel and needed time off for the children and coaches as well as ongoing coach training.
Calendar: We will provide you with a complete calendar including scheduled vacation times. We provide coaching 46 weeks out of the year. Scheduled breaks are included to allow for travel and vacation. All breaks are built into the schedule and no payment adjustments will be given for vacations taken outside of these designated weeks.
Payments: I understand that a registration of $50 is submitted with this form. I will receive a link via email to a history form which must be filled out before starting coaching hours. Upon submission I will then receive a phone call from the Brain Sprints offce to set up my coaching hours. Monthly payments for coaching hours will be processed on the 1st day of the month and start the month coaching commences. If coaching commences after the first of the month, the first months payment will be prorated. Your first year includes three assessments and three changes in the NeuroDevelopmental Program. Reassessments are scheduled very four months and will include a parent consultation after each one. These fees are included in my monthly payments and will be automatically withdrawn from my debit/credit card.
I understand many of the neurodevelopmental materials needed to implement my child’s program in the center will be provided by Brain Sprints. Any supplies needed for home use will need to purchased separately.
If you decide to withdraw from the program, you must provide 30 days notice in writting to office@brainsprints.com prior to the date of your next monthly payment. Payments are non-refundable once processed.
Cancellation Policy: If you are planning on discontinuing your program, we require 30 days notice. Your coaching hours will end at the end of the following month and withdraw your child from the Brain Sprints program. If you decide not to return in the month after you submit your 30 day withdrawal request no refunds will be issued for amounts paid.
Emergencies and Illness: We realize that, at times, cancellations due to family emergencies or illness is unavoidable. Thank you in advance for not bringing a sick child (fever within the last 24 hours) to an evaluation or coaching session. We will work with you to reschedule your appointment as soon as possible.
Lost items: We will have a lost and found bin in the office, please check for lost or items left behind. The lost and found will be emptied at the end of each month.
Damage to property: You will be responsible for any damage to the building or Brain Sprints property that you or your family cause. You will be charged replacement value for anything that is destroyed.
