Hey there! Thank you so much for showing interest in working with me. This form is meant for me to learn about your skin. This form is the first step in our skin journey together. Please fill out these basic questions and give me as much detail as you think I will need. I will use this form to learn about the services your looking for, what your skin goals are and if you and I will be a good fit to work together. When I have the chance to analyze your skin in the spa I can create a much more accurate skin goal by recommending what other services to incorporate if your interested. Your answers are completely private and confidential. Also know that I am so excited to begin a great working relationship with you. *Let's get something very very clear.. your skin, skin-care is not a luxury. It's a necessity. Some people get facials for relaxation, others get them for healthy skin. Whether you're just needing time to relax to add a bit of peace or a full skincare regimen because that's my job. You require a committed investment to take care of your skin. After that, you require maintenance every 3-6 weeks, which carries a separate skin goal and peace of mind investment. Please be sure that you are prepared for this.

The price can range so greatly based on added recommendations, products needed, and short or long term treatments, ...etc.

I will do my best to give you an accurate price estimate. Please know that the price can change when I see your skin goals. I cannot guarantee a price quote until I see your skin in person and can confirm the price I have quoted you based on this form and when I see your skin.
Please allow me 24-48 hours to get back to you or if we're in person I will give you an immediate response. I will always get back to you as soon as I can. In most cases, an in-person consultation is the best option for creating a final skin renewal goal. However, I do understand that we are busy and an online consultation can save time. Please know that treatments won't cure medical skin conditions. Skin can look very different in a picture than it actually is in real life. I will put together a treatment plan and a Skin Renewal Loyalty Sheet based on the Consultation Form. I will do my very best to send you accurate information but this is always the risk with online-consultations and face-to-face.
