I acknowledge that I have received a copy of the VSC Resident Handbook, including the Sexual Misconduct Policy and the VSC Policy against Discrimination, Harassment, and Retaliation and had the opportunity to review and ask questions about them.

I fully understand the contents of the Handbook, and agree to abide by the guidelines and policies stated in it.

I understand that if at any time my actions jeopardize VSC or are detrimental to the well-being of the residents, staff, or the Johnson community, the Executive Director may give written notice to me demanding that such behavior desists and/or requiring that I vacate the premises and terminate all participation.
